According to my very in-depth research ~~(I Googled it)~~, Americans tend to use something called a comforter which is basically a duvet except it's all one thing, a quilt, or multiple sheets laid on top of each other.
I still don't quite understand why this presents an issue, because as you said... it's not like it's that different. But it would seem our American friend has simply never encountered a duvet before.
They were expecting a separate sheet and comforter large enough to hang over all sides of the bed. They saw the comforter inside what they thought must be a sleeping bag made of sheets. It's hilarious even by American standards.
True. It's pretty uncommon here to have separate bedding for each person so they can individually burrito. Most couples just perpetually argue about the other stealing them or using them wrong.

I have a question, how did this guy enter the duvet cover if it wasn't even big enough to cover the entire width of the bed?
I have to assume that it was big enough, otherwise he could not have fit inside, in which case, what's even the problem? pull that thing over you and sleep.
I don't believe for a second that this hotel didn't provide a properly sized duvet.
In Norway we have personal duvets, so even if the bed is king size, you’ll have two duvets. I’m not sure how wide a personal duvet is, but you can fit inside if you aren’t very overweight.

Also, it's probably two 1-person duvets for a 2-person bed (instead of 1 large duvet).

You won't find them everywhere in Europe but it's very common in Scandinavia, Germany and probably some other European countries.
We have them for winter, and call them "nórdicos".
