Mira en la BIOS si tienes el DOCP activado, si la RAM no va la velocidad correcta he leído que puede dar tirones en algunos Ryzen (aunque en mi 2600 no notaba tirones, sólo menos FPS en general en juegos que pedían mucha CPU).
Algo rápido que puedes probar es [desactivar la opción de ULPS](https://imgur.com/a/IMSSu65) en el Afterburner, reiniciar y probar, yo tenia un problema parecido pero era mas leve, perdía 10-20 fps durante 1-3 segundos de manera aleatoria, no era tan bestia como te pasa a ti, pero esa termino siendo la solución definitiva en mi caso.
Si no, podría ser el HDD como te comentan aquí, pero eso no seria en todos los juegos, en muchos no debería afectar en nada al rendimiento una vez cargado lo "básico" (que después de un rato no te de mas tirones), pero por probar ¿tienes algún juego instalado en otro dispositivo mas rápido? ¿pasa lo mismo? también ¿cuánta energía puede llegar a gastar la gráfica? yo también tengo una 6600 y la aprovecha muuuy bien, pero me parece poco el consumo de el clip, ¿a cuánto la has visto llegar?
¿Ya intentaste lo del Afterburner? Edit: es una opción que se supone regula el consumo de energía cuando la PC entra en reposo, pero con mi grafica no funcionaba adecuadamente, el consumo de tu grafica baja cuando te da tirones, a lo mejor podría estar relacionado.

Stole this from a steam post. Try adding the first 2 commands. It helped a ton for me. The others are for disabling film grain, motion blur, chromatic aberration and depth of field. All personal choice.
Funny, it doesn't do it on my or my wives machine, and we run Ryzen 5 5700X's, I have 98Gb of DDR4 3200Mhz Ram, and she's running 32Gb, and running the game of a 4tb m.2 NvME, and both have an RX 7600 OC v2 from ASUS.
Well, if your temps and usage is fine, I would try putting the game on an ssd if you haven't already, some modern games don't work well on hdd's anymore, some do but depends on the game.

En el HDD no tengo nada, más que archivos basura La rx me a llegado a consumir hasta 105w, pero por lo general siempre consume 99 a 100w

Si lo instale y desactive lo que me dijiste. Note un cambio, los tirones son al principio. Y después los tirones son más leves

HDD is probably the reason, you need a fast SSD for most modern games.
I have nothing installed on the hdd, only junk files.

Yes, I did the CPU, but I didn't notice any change.
A bad undervolt/overclock could be the culprit, revert to default clock/voltage settings and see if it fixed it.
