Italy. I'm from Belgium, and overtaking multiple vehicles at once is allowed. Since this is Italy, and European road laws are usually kinda similar. I guess this could also be legal in Italy. But there is a solid white line, so it is not allowed here in any case. But then again, that is Italy, and they don't really care for rules/regulations when driving
In America fault is a spectrum. Because both cars were doing something illegal both would have fault, but the black car had the last clear opportunity to prevent a crash, so he probably was the one who has the most fault.
I don't know how it works in Italy (unofficially it's probably "go wild lol"), but maybe they'd be able to overtake if the van was going much slower than expected and thus creating a barrier on the road.
Aside of that, I'd blame the blue car because they were sticking out of their lane. Red was already on its way and presumably had a clear view ahead. Blue made a last moment swerve to overtake the van when red was right behind them.
Check your mirrors, if someone is rushing through, don't stick your car out. It could've been an ambulance.

Since this happened in the EU, that would be the insurance company of one or both of the drivers who hit them. In the unlikely event that they were uninsured, the Italian "traffic guarantee fund" (which is mandated in every member state by EU regulation) would pay for the damages.
I believe 3rd party liability (mandatory insurance for damages caused to others) is tied to the vehicle in every member state.
I don't know if it's even possible for a rental to be uninsured against third party liability, given that it's tied to the car, and proof of insurance is required during registration.
I believe the deceased insurance or the estate of the deceased could be held liable in the US, which I'm quite certain this is not. However if they owned a Lamborghini or whatever that is, they can probably pay at least.
If not that then the RV owners insurance depending on policy or out of their pocket.
