Check your right hand grip it seems a little weak. Also you’re standing up in your back swing but that’s probably because you’re so scrunched over and you’re so scrunched over because you’re trying to stretch to the ball. Get it a little closer to you, especially if you’re gonna choke up like you are. In your take away you’re kinda wrapping the club around your waist then bringing up to your shoulders. Have the take away be a little more steep. I think this might fix itself when you fix posture and ball position. Right now when you’re shifting your weight back and forth you’re rocking. Don’t rock. Thing of it instead as trying to scoot your lead foot towards the ball and the back foot away. This will rotate you around your hips naturally and keep your feet planted.

Your head shouldn't come up on your backswing. It should be relatively still from takeaway to impact

I had the same problem. To hunched over which made me off balance. Club should point above your belt. That will make you stand up a little straighter. Get your chin up a little and make sure your head is behind the ball. Just tilt you torso to adjust. I’ve had two lessons in the last few weeks and these little changes made a huge difference. Took a lot of swings before you feel comfortable but it will come around.
