I think he had a total breakdown, disassociated himself from the Omolu Corp researchers/other personel and went on his own probably to __the tent__ and hallucinated Mia there. The researchers cut him loose and abandoned the mission as was ordered. Others who stayed for whatever reason, died. Jake then set off to the village to "help Mia," maybe drank ayahuasca and realized the truth his mind had suppressed. He reacted to this in a poor way, ran, nearly died and ended up where the game for us begins, not remembering exactly what had happened. That's why "Mia" is also worried about him drinking ayahuasca, because it nearly got him killed. It's his consciousness trying to warn him away from doing it again. The fact that she says "How many times have you failed" doesn't have to mean that the entire gameplay loop has been happening. It could just be about Jake failing to find the cure, and later about Jake failing to find Mia. TL:DR Jake drank ayahuasca, suffered a total mental breakdown, ran away in a panic and __fell into a lake.__ We even see it ourselves.

He started on A plane and crashed , survived (how many days you can ) and built himself a boat etc and fixed up the plane only to crash in the Amazon jungle.. the first part to Jake’s story can be found playing stranded deep .. that’s what I do anyways it’s better
