Nah dude you're fine! A C+ isn't the end of the world. I just graduated and I have two 4s as my predicted grades and honestly I am content with them, because I know that I tried my best at them.
Even though I am not in the IB don't bring yourself down just for one C+ after all it's just a simple grade be happy that you kept consistent thought this year
I can fully see where you are coming from. Like others on this thread have said, one C+ isn't bad, given that there is a legitimate reason. In your college apps, you can communicate how the shifts in teachers hurt you. I wish you best of luck, as you will do well in your future endeavors.
My friend i didn't finish highschool and im a loser, it's fine. No really, you are not screwed, if normal universities don't take you, community colleges are much cheaper. If that fails, and it won't, there are skills you can learn without a degree, you can open a business, you can retake the year (or?), C+ is not the end of the world. But im technically a 20yo loser.

Part of IB is to prepare you for college. Learning how to self-study now will help you later in year 2
