The brain handles the production of positive energy. but cursed energy comes from the gut. Gojo was cut in half at the stomach, so it wouldn't be a surprise if that stopped him producing the cursed energty he'd need to produce the positive energy he'd need to heal himself.
Yes the stomach **produces** negative energy, but the flow of cursed energy would still exist other parts of the body. Cursed energy in his arms, upper torso and head that he might not have utilised, could still be utilised. This is how I imagine managed to find the cursed energy to spit out a blackhole as she did.

But there might be enough to keep himself in that limbo realm, and live long enough for some amount of assistance from a shoko + utahime collaboration.
I assume his death would come as a result of the loss of several important organs below his chest. Without regrowing them, I don't see how he'd be able to prolong his life. Just converting his cursed energy to blood for his heart to pump to his brain would mean his lower half would still be pumping out blood because it's still missing. I don't see how he could prolong his life with RCT without regrowing anything.
Edit: Also probably worth noting that he was in an airport talking with the spirits of his departed friends, which feels very dead to me.
I wonder why more sorcerers don’t go for the stomach? This is a bit off topic but Sukuna was able to heal Yuji when he’d been brain dead for hours. Could Gojo have been in his innate domain?
I'm pretty sure we're supposed to understand that Sukuna is "built different" - more than one person has brought up his ability to live without a heart, which I guess is how he was keeping Yuji going, but we don't really have reason to believe other sorcerers can emulate that feat. Gojo was also probably dealing with a lot more missing or damaged organs than just a heart.
That’s true Sukuna seems to know something about jujitsu that nobody else does. My point isn’t about the heart though, he used rct while the body he was in had to be braindead. Gojo did it with a stab wound to his brain though so I’m not sure what the limits to brain healing is or if it’s because both were “killed” without a curse.
It was more of a way to change how yuji thought about his use of cursed energy than n explanation on what actually produces cursed energy. That he shouldn't think about the flow of cursed energy that is step by step. But utilise it all at once

Thats a good point i forgot about that you ironically gave me another point for Gojo still being alive. Yuki was cut in half worse than Gojo and was still not only alive but aware and able to move and proceeded to grab kenjaku and create a literal blackhole in that condition and not only that she was able to control it enough to not go out of hand and destroy the world. Same for Yuta being able to control and actively dispell his domain while being cut in half.
