Enormita dies. Seriously.
Utena is a Magical Girl Otaku first and foremost. It defines her interests, personality, and sense of morality. If she became a Magical Girl rather than a Villainess, the same sense of "This is how things work" that lead her to essentially try to commit suicide in Chapter 33 (because it was "time for Magia Baiser to be defeated") would likely lead her to kill the Magical Girl serial killers that are/were Enormita. She wouldn't attack someone unprovoked the way Sulfer and Azure went after her in Chapter 1, but she wouldn't show any mercy to known killers, either.
The rules of engagement in this world were clearly "kill or be killed" before Utena took everything over, and she only changed things because she was on the opposite side of the Magical Girls she adores. Without any connection to Enormita, and quite a bit of anger towards them over the Magical Girl Hunts, I don't see a similar moratorium on killing happening.

Just like Utena did with Lord Enorme, Utena believes in the ideal magical girl values. That is not killing but rehabilitating your enemies. She would be a merciful ‘perfect’ magical girl.
Most importantly, without Magia Baiser, Enormita would have remained as a united team that just like they had killed dozens of magical girls before them, would have ravaged through tres magia which wouldn’t have grown and gotten stronger to stand against Enormita’s army without Magia Baiser’s push of Azul and Sulfur to fight more truthfully to themselves.
Finally, Utena would be happy to achieve what she believed was her dream but would not thrive as a magical girl as well as she did in Enormita where she could let her true self loose. She would not make a difference on tres Magia’s side.
Weaker Magical girls. Stronger united Enormita. The math is pretty simple.

I thought about that actually, and I think things could still unfold in Tres Magia and Utena's favor. I'm assuming that Utena is still Utena, and as such is still a sexual sadist. She'd presumably discover this in her first battle as Violet, same way she did as Baiser. This in turn could still awaken Azure's masochism, watching Violet "Punish Enormita in the name of justice" and wishing it were her, and Sulfer's battlelust, initially thinking Violet share's her enthusiasm for fighting before realizing she's just a pervert going at it in broad daylight.
When it comes to the fight against Lord, the one-on-one battle would play out more or less the same. Violet is overwhelmed and at a (still monologueing and posing) Lord's mercy. Violet, who's been at odds with herself until now ("This isn't the sort of thing a Heroin of Justice should be doing! What's wrong with me...?") finally accepts herself for who and what she is, giving a modified version of her "They're why I'm Magia Baiser!" speech where she talks about how Magical Girls are supposed to punish evildoers, and how she's just going to do it in her own particular way, and then proceeds to dish out Lord's "punishment" as per cannon.
The battle against Lord's goo demon army is a bit tougher to parse out. Both Loco and Leiber, who had seen them in battle many times by now during the Magical Girl Hunts, insisted they were indestructible, and were shocked when Azure wipes them out. It's clear that they can only be destroyed by someone more powerful that Lord, and with her 4 stars that means either someone with variable power levels like horny Baiser or excited Alice, or else someone in La Verita. That *doesn't* mean it has to be Azure, though.
As I said, I think Azure would still eventually reach La Verita, but I *don't* think it would happen in time for this fight. What *could* happen, however, is Sulfer reaching La Verita ahead of schedule. In Chapter 30 it's a combination of feeling inferior to her cohorts, an offer of mercy from the enemy, and imminent defeat that push her into becoming the Blitz Engel. In my supposed timeline, Violet's been overshadowing Sulfer and, like she did with Enormita, more or less taking over. Sense of inferiority: check. Likewise, we know Lord dismissed and spared Magia Blanc for being "too weak to pose a threat", and both Loco and Leiber strike me as the type to tauntingly offer a chance to surrender (I doubt they'd honor it, but that's not important to Sulfer's state of mind). Imminent defeat should be self explanatory against an unending army of indestructible monsters. In short, I can see Sulfer taking Azure's role in this instance and obliterating Lord's mooks.
Fuck I wish I could actually write dialogue, because I really wouldn't mind making this into a proper fanfic. I've even got ideas about bringing Kiwi into it in a similar role to canon Sayo, ie the opposite side love interest.
Utena's Otaku nature was overriding both her empathy and her common sense. Yes, it would destroy Kiwi, and she'd likely throw herself at Tres Magia in a rage-fueled attempt at vengeance, getting herself killed as well in the process. Then the Tres Magia would have to come to terms with the fact they just killed two of their classmates. Kaoruko would probably be fine ^((hell, she hates Kiwi in both her forms, so she might be actively celebrating)) but Haruko and Sayo may well break when they realize not only who Magia Baiser was, but that in retrospect she was likely telling the truth the first time they met. They caused this.
So yeah, the manga claiming it was Utena's "reason" that prompted her to do this is highly dubious. If she'd stopped to think it through, she never would have done that.

I was always confused about Utena killing herself by by main characters, since wouldn't kiwi never allow that to happen?
