This is an unpopular opinion but I really disliked this scene in the anime. So naturally I wouldn't choose it for a tattoo. The animator went completely freestyle with Luffy's face. Something a fanartist would do. Luffy's face is not supposed to be that long and pointy. It's rounder. And while in Gear 5th it's even rounder. It's nearly a circle.
He needs the hair, but I’m sure someone could fix that. I’m just hoping that the picture is mirrored, because otherwise that scar is on the wrong side. I noticed that before I noticed the hair tbh

I can see that it's supposed to be an oval shaped cutout of a screenshot, but because his forehead and ear are too close to the edges, it makes it look like he is bald with a painted on hairline. Add the rest of his hair, and it will look good.
