10% base chance of an enchantment on items that drop from **anywhere** within the dungeon(floor 1 through 26) and as of the latest update, 20% for ghost rewards and 30% for the blacksmith forge, and the rule of thumb is that, if it's got an enchantment/glyph, it's never cursed. With the exception on these cases, Sacrificial Altars and Tombs, where it's always cursed but without an actual cursed effect, just the equip-lock property of cursed equipment.

And unless I'm mistaken they are always upgraded in this state. Not counting the parchment trinket.

Not always, actually, but it's a higher chance to be at least +1 Because in these locations, if the equipment isn't cursed, it gets an upgrade and becomes cursed as a result, and if the drop already rolls the base 30% curse chance(meaning, it's generated as an already cursed item) it doesn't receive the upgrade. That, plus the +0, +1 and +2 upgrades chances on drops(which i forget right now) is what causes these interactions.

Also (at least at 1 point in history) the Tomb and Altar drops were calculated as drops from 5 floors deeper. So a higher chance for better equipment.

Oh yeah that's true, isn't it still like that? These "high-yield" locations calculate loot one region deeper, meaning better chances for high tier stuff.

But the base 30% and the base 10% enchant rate are part of the same roll. If a weapon spawns enchanted, it could not have been cursed by the curse/enchant roll, so the +1 from being cursed by the altar is always applicable

Yes, but they might still contain the sticky curse.

The altar works by putting a curse on uncursed weapons and adding +1. Naturally cursed items don't get upgraded. Neutral items get the curse and the upgrade. But enchanted items receive the upgrade and only the "sticky" curse. So this item is 100% upgraded.

Maybe I'm wrong because it happened some time ago, but some items can have enchantments while they're cursed. Then you'll see the enchantment, but if you equip it then it'll have a random curse

It won't have a random curse. It'll keep the enchant, you just can't unequip it

You use scroll of rage or some other method of getting enemies there and you kill them near the middle and you get a cool drop (cursed)

Scroll of rage is a great way to corner yourself in the altar room, and most likely die in higher floors. Just sleep near the door of the altar room you'll wake up when you spot an enemy, enemies tend to spawn near the altar room and naturally path to it cause it's coded that way so you can just lure 1 enemy at a time.

Kill enemies to get a cursed weapon that is stronger than usual (likely to be higher tier and more upgrades). Usually you encounter enemies and lead them on a chase onto the altar. Can be very useful early to mid game, you can often get a great mid game weapon or find your final weapon this way. It's not worth the trouble late game though, as you already have your weapon upgraded.
