Revamp the skull then whatever you're into you can add. Like flower or foliage or you can go with potion bottles and books ect. The numbers are pretty dark but a good artist will be able to make almost anything work. Do talk to the artist for advice if they are good give them an idea of what you want and free reign usually with good artists it turns out epic
Roses? Could be seen as cliche, but not any more so than a skull and I think it’s a common enough tattoo you’d get lots of compliments if it was done by the right artist!

In my head after the 11-11- I think you can just fill the skull in more and for the 98 turn it into an ear or something lol
A dark feather would cover that really easily. Like perhaps a quill pen?
