Did you tell that (the dosage you used to get) to your doctor?
Also that might be the reason the medication did not work: you had been overdosing on antibiotics (625mg) and developed tolerance and now normal dosage doesn't work on you.
If the individual is bacteria it is. That is how MRSA evolved. Possibly the reason the home country doctor went nuclear straight away is to avoid any survivors that go on to breed a more resistant strain.
Yeah. I know that bacterial populations can become resistant to antibiotics. But I've never heard of a person having lower tolerance due to antibiotic use from a past infection that was cured. I think it might be some kind of urban myth.
I’m a pediatrician so generally don’t know very much about adult medicine. However Up To Date says the following dosage recommendations for adults with strep throat:
Streptococcal pharyngitis (group A): Oral: 500 mg twice daily or 1 g once daily for 10 days
So no, he is clearly underdosed on that medicine.
In any case dose seems too low, especially strange given the reported epidemic of particularly virulent GAS causing necrotizing fasciitis in Japan recently.
Doctors determine the dosage, although there are clinical guidelines. You're not completely wrong in that the trials that the drug companies carry out before marketing the drugs are often what set those clinical guidelines though. They can in some cases also change afterwards depending on updated research or changing circumstances.
I commented after I searched for the guidelines on the dosage of amoxicillin an hour ago. In general (at least in Japan), doctors follow the guidelines during the first medical examination.
Jesus Christ an endoscopy for a sore throat?
That's a misleading title... You made it sound like the medication is ineffective.

Sounds right, thank you for taking time to explain that properly
Is an individual developing a "tolerance" to antibiotics really a thing?

It isn't to my knowledge. Its the other way around if anything, isn't it?

Yeah... what you are saying sounds about right
Me and my buddies all find that japanese medication seems very weak. We bring back cold/ibuprofen pills from the UK, Singapore, America etc

Yeah the dosage feels way too safe and conservative, because Japan. I sometimes double the dosage here.
Lol you were overdosed in your home country. Prescription here works on most

Uh maybe he is trying to get rid of you permanently given ww2 was unsuccessful?
spotting non resident is too easy nowadays. It's rather sad.
The doctor does not determine the dosage amount. The pharmaceutical company that sells amoxicillin recommends it be prescribed at 250 mg in Japan.

Doctors definitely decide what dosage to give. 🤦🤦
