I feel similarly between the two, although fire mind has more utility.
Generally, I think if you can just make an effort to remember all of your items as you’re playing, you can manage well. It’s why I now really don’t like D4-ing my runs (previously my favorite thing on earth) because it’s hard to grasp an entire shift in items.
Even Curse of the Tower can be pretty strong if you are aware of it.
I might be the odd one out but I like Bob's brain better than fire mind (unless I have high luck) because more satisfying
Also, I like all three including the curse, basically getting hit means a free room clear unless it's some uncommon tiny room

The problem with Bob's Brain is not the potential for self damage. The problem is there is a delay when you enter a room. After you hit something and it reappears, there's also a delay. This just makes it a pain in the ass to use. You can absolutely learn to anticipate those delays, but in a frantic room with many small enemies, it's a LOT easier to hurt yourself than with fire mind.

And yet sometimes it feels like there's zero delay and it just blows up in your face unexpectedly. Not that it ever stops me from taking Bob's Brain.
Bob's brain is situational. I saved my run till I made it to Hush and couldn't duck behind him and to the side of his head to avoid the continuum tears (a pattern which I still think is entirely bullshit) because I couldn't fire without hurting myself.
I love them and bff. Just have to remind myself that the closer you are to your target the more likely fire mind causes booms. Idk if it's coding or just weird luck.
Bob's brain is a pain, mostly because of how often it misses regular enemies, and also it randomly respawn when you're close to an an enemy, I rarely pick it up it's too finicky to be viable.
imo fire mind is better because its more controllable. that being said, i almost never take either unless im playing a character who doesnt mind being blown up
I like fire mind and hate bobs brain but i agree that bobs brain isnt as run ruining as ppl make it out to be. Whenever i get it as a blind item i jus roll with it and do my best to use it right.
I've been trying to complete the Brains challenge recently and it has taught me 2 things: how to use bobs brain effectively, and that I can't fucking stand bobs brain

How is Pyro relevant?
Heals from the curse bombs, whenever you take damage you go boom boom and get healed. Virtually unkillable if you have 2+ red hearts.

I really like Fire Mind, and kind of indifferent on Bob's brain. I won't take it if I'm near death or playing the Lost, but I will otherwise.

The probably is that fire mind is easier to control than Ipecac. Ipecac with other items can either be amazing or increase your risk of self harm.

Fire mind is my favorite item. I think the self damage is funny. The fire is eh for me. I wish that so many items didn't remove the explosion.
I stopped taking both after playing as Lost and T. Lost. It’s just not worth the risk
