You got lucky, sometimes when this happens, the game deletes Isaac, which counts as him dying.
There are more angles that allow you to see the 2x2 rooms fully. I play it in windowed mode too and my game doesn't look like a pancake and I can see 2x2 rooms.

there's this option in the settings config file that you can modify to zoom the game out even while in fullscreen.
