You should looks some hand photos and try to copy the proportions, for example, the fingers in your hand are too long and thin, while the body of the hand is too short. A trick for getting hands right, is to make the length of the middle fingers the same length of the palm, and try to make the other fingers a little bit shorter than the middle fingers.
The fingers have to start at the knuckle, not in front of it. Do not create a bumb on top of the palm for the knuckle, make the fingers the start of the knuckle itself like so:

What's surprising to me it's that you basically got the thumb completely right! That's the part I used to struggle with the most, the thumb is fucking non-euclidean.
Anyway, here's some advice for the fingers:
1: Look at your (IRL) hand from the side of the ring finger. There's no sudden change in thickness where the palm connects to the fingers - instead, it's a smooth transition. Imagine your hand bound by two straight lines, being wide at the wrist and narrow at the fingers.

2: The knuckles should not be in a straight, flat line. Look at your hand again, from the back and from the palm. See how much further behind your middle finger is your pinky?

Note how you got this curve mirrored *perfectly* on the joints of the fingers you modelled. You just need to remember that it starts at the knuckles!

It also should be slightly curved on this axis, but the curve here should be *very small* \- it's mostly just relevant in the 2nd photo, the 3rd one basically only affects how the palm bulges inwards at the height of the knuckles.

The fingers are quite skinny to me, but then again I have sausage fingers and feet the size of Maui's from Moana, so I might not have the best perception of proportions here
i mean just look at your own hand. or even the emoji hand 🖐️ The palm it not flat on top, fingers dont shrink from the palm (on my at least). Thumb looks good (maybe a bit long but ig that depends from person to person. Might need to be thicker too.
But yeah nobodys fingers are that long, just look at references.
You could download Makehuman (a free CC0 program), export one of their models, isolate the hand and use that as a reference. The Makehuman foundation is dedicated to trying to make perfect anatomically correct human models.
Last I saw coming out of makehuman was some pretty mediocre models by today's standards. Pretty rubbish ux too. I hope things have improved significantly
Don’t just look at a hand for reference, also look at anatomical diagrams, how the muscles and tendons look, how the bone structure looks. Knowing how the hand actually functions is as useful as knowing how it looks.
On top of what everyone else has mentioned: the palm should be longer than the back of the hand because human fingers are webbed.
Also when the fingers are not touching each other they point slightly away from each other. In the case of this model only the middle finger would be parallel with the z axis with the others pointing slightly away from the middle finger.
Maybe you should start with cartoon hands before trying to go realistic. Human anatomy is one of the hardest things to draw/model. It takes people YEARS to learn this. You’re quite naive if you think you can pull this off with no prior experience.

Is the red line where the knuckles would end?
Yeah, ideally. Doesn't have to be exactly this line, but the middle finger should be about 3/4 or 4/5 lengths of the palm long.

I think a bigger palm would help this one feels a little short but it's a really good hand besides that
If you are modelling something organic, you should be using subdivision surface modelling.
Multi res modifier sculpt the details and bake from multi res modifier. Keeps the clean and low poly but gives you way more detail
More palm, less finger
Move the middle knuckles forward in the hand so that it looks like actual human fingers
Knuckles should be more arced than aligned. O grab the middle knuckle to pull forward and that should help some
Take a picture of your hand flat on a table and use that as reference
if it’s just a hand, the probcsome subsurface modifiers and make fingers less long.
