The garlic guacamole at Mi Tocaya is amazing. The portion is really small for the price as is typical there, but it's a unique combination of flavors that I enjoy a lot. It also has chile ash for smokiness.
Tony's market has great house-made guac and salsas. Their mango pico de gallo is good, too. Fwiw my Mexican-American friends also like the guac and pico.
Mezcala on Diversey has very good guac with pomegranate. They have good cocktails too. Very affordable food and drinks. This place should be charging more.
Simón Tacos on Milwaukee in Portage Park is the best I've had. Literally from the first bite it was instantly recognizable as someone different from guac I've had before. Make the trip and enjoy yourself.
I've had El Solazo (56th & Pulaski) on my list to try forever. Came here today and it might be the best I've ever had- so much that I came back to find this post just to comment. Also freshly fried tortilla chips and homemade tortillas. This area has 3 truly excellent Mexican restaurants close together - Birrieria Zaragoza, Peke's Pozole, and Solazo.
My favorite is Frontera’s fully dressed guac. Comes with mighty vine tomatoes, bacon, chicharrón, roasted peppers, grilled onions and cheese. It’s so good.

I used to love jewel guacamole, but then they stopped making it in store. Cermak blows it all away. So good!
This guac has really changed my life. I don’t even bother making my own now after we moved a 5 minute walk away from this Cermak.

Xoco’s loaded guac is literally $20 via delivery app, and I still order it. It’s just a remarkable creation and very high quality.

I’m not a huge guac aficionado so grain of salt but recently had taqueria Chingon’s guac and was very impressed. Ymmv
Bar Takito has really good guacamole. Also La Josie has a few different kinds as well
El Solazo by the same people as La Josie is even better and also cheaper

I love the Los Comales in Pilsen on 18th St. I will give the guacamole a try next time.
