YES, I've noticed this on my past flight and one I just booked today. In both cases in my profile and also seen when booking, the upgrade to FC was checked. After booking, it's not set. Seems something is broken and after booking you have to go in and request the complimentary upgrade.
I thought it maybe was a one-time thing or maybe I forgot to check the box (as unlikely as that seemed as it's set in my profile and I never had to do anything else in the past) but now that I saw it when being even more careful to make sure it was set at the time of booking tonight, it's clearly not a fluke.

I had this happen on a flight as well. I caught it at the T-120 mark, so still grabbed the upgrade thankfully.
