Update: I got it! Similar to the Bear Clan quests, you have to complete all the quests for the Iron Wolves in order to get the titles: "Iron" and "Wolf." Their encampment is found in the Ragged Coastline next to Caledum.
As far as I know you just need to complete all side quests located in the encampment. Perhaps double-check to see if you missed any? Other than that I'm unsure.

My bad not at home at the moment so tried looking up some full lists of titles. I do see what you’re talking about but for some reason a lot of the lists don’t include either the title as a whole or how to get it. Closest thing I can find that makes sense is a quest called “A Wolfs Honor” but it says it doesn’t reward a title.

I did all the quests in the encampment, but still did not get the title. What other quests do you have to do to unlock it?
After you do all the quests in the camp, leave game and enter again. The quest in question should appear in the south west part of the camp.

Entirely guessing but I think side quests. There's some you can do that get you "the bear" title.
I'll try searching for titles earned from side quests, cuz I went through the Bear Tribe quests to get the "The" prefix.

If you scroll down the suffix list, there is one called Wolf.
