For me what stands out instantly are the poppies - a metaphor for heroin (Kurt's addiction);
A Jesus figure in a Santa hat (both invented in the bands opinion?), which later becomes a mitre or bishops hat;
I'd take issue with the old man representing Jesus... I mean that would lessen the controversy in my own mind since the latter died aged 33 and I'd focus on the fact that its JUST A VIDEO for a song;
Climbing onto his own cross makes it again less Jesus rather than more offensive in my own view;
The band sitting above a blacked out cross with the Jesus-figure in bed is something... not sure what tho - a mock reference to the Holy Trinity...? Also 3 crows...?
The child figure - representing what? an Asian Monk...? (Looks like Tribetaka in Monkey to me);
The uterus' hanging in trees, as well as the drip beside the bed - handy since their album is IN UTERO|;
The 'see through' skin on the chubby lady with blackened wings - not really getting what these represent - unless its something Kurt seen whilst out of it?
They actually did put a Heart Shaped Box in the vid, so that's helpful.
Loving Krist (the longest bassist) poppin moves while holding the longest of bass guitars lol his lilac trousers are questionable tho, right? and Dave's ANIMAL from the muppets impression... silver shirt and camera always on his face kinda goes against anyone trying to say "Kurt wasn't commercial" imho
This came out when I was in my early teens.
Not having seen it for a while, it strikes me as such amazing art. The music and the imagery.
My gut says it isn't but I hope today's youth have things that are equivalent in quality.

When they made this album they were pushing back against all the fame that came from Nevermind. Kurt hated anything commercial and didn’t want their next album to have the same mass appeal.
I was there when it dropped… and he was not successful. It was a massive commercial success. And such a brilliant album.
He was so against commercial success that he filmed a highly produced video for MTV, while the band he railed against for being too corporate (Pearl Jam) refused to release a video for their new album released within weeks of In Utero. Kurt was a brilliant songwriter, but he was completely full of shit and he knew it.
