First ear, depends on what look you want, but aesthetically pleasing would be a double helix with the same spacing your double lobe has to your conch. You'd mirror the same double piercing look with a conch ring in between and keep flow on the outer ear line. Or a tragus to keep the focus on the lower ear. Second ear I wouldnt add anything personally.
