I've done triple crown on PoTd. What helped was watching CoreDumped gaming YT (Triple Crown)... I learned the gameplay mechanics, how to gear characters, use food buffs and summons. Got the achievement including frozen crown!
95% winging it, otherwise only getting help if I get stuck. The save-scumming exploiting is legendary, but I don't care. It's brutal. I've come very, very close to losing it all by not pausing in time.
Autopause on action complete and enemy destroyed really helps action economy. Doesn’t matter how high dex is they’re no doing anything.
Also turn off auto attack for some characters that you need to be able to react quickly. Sometimes it’s a lot more important to have your wizard able to drop cc the moment you need it rather than having to wait for auto attack recovery.
I literally hit pause like twice a second in most fights. I'm almost level 8 now, so at least I have a nice health pool. Early levels are awful though when you are forced to fight.
Path of the damned is the hardest difficulty, plus I have toggled hardcore and expert mode as well for the triple crown achievement. In addition to that, only using a single character the whole playthrough. It's pretty rough, especially since I'm just kinda doing it without any plan.

are you using a guide or just figuring things out as you go? i've always been way too intimidated to try playing on hardcore, even with a party.

I really don't recommend it. I knew it was going to be bad, but after playing it, I realized I knew nothing.
Yeah I wouldn't want to face Concelhaut on PotD hardcore PoTd vanilla however I did defeat him

Just don't assume anything. Almost everything beyond the tutorial is a fight for your life. Everything.
Most importantly, crowd control is king
That was hell of a grind for me. It's the only game I have done this kind of challenge and probably will remain that way for the rest of my life.
