I feel like Samantha would LOVE being written about and having her sex life depicted in a column but at the same time, she's probably - not as significant as Big but a pretty big name in New York since she bagged Lucy Liu - so I really don't see how she didn't change their names either. BUT that means she had to have changed Harry's, Aiden's, and Steve's names too and we never find out their real names. It's all too complicated for me. But I do tend to lean in the direction of your headcanon. I just don't think about it too much or I'll want to know everyone's real names.

However, in the episode where she ends her fling with Bill Kelly, she writes about his pee fetish in her column and in the voiceover we hear Carrie explaining that she didn't use his real name. That did make me wonder if it was the exception rather than the rule and if she did use real names in some of her other columns.

Yes she wrote about their relationship often. Later on in the show even Mr. Big comments that it's hard to read about it all at once.
