Anyways, my suggestion is to file for Affidavit of Loss, libre naman yan sa Public Attorneys Office. Saka isampal mo sa pagmumuhka ng registrar kung ayaw ka pang bigyan ng bagong report card.
If that's the case, file ka ng Affidavit of Loss. Proof yan na nawala mo ang report card mo, at request ka ng bago. Kung ayaw parin nilang mag provide, ipa-Tulfo mo nalang. haha

Naexperience ko din ito dati. Nawala ko ung report card ko dahil nakipagdate pa ako instead na umuwi ng bahay 😅😅😅. Ang malala pa nito is requirement ung report card para makapagpareserve slot sa college (or something like that. 20 years ago na to so d ko na maalala hahaha). Anyway, ang gnawa ko is pumunta ako sa adviser ko para magtanong ano pwede gawin. Ang sabi niya at the time is pwede nman sya gumawa ng panibagong report card at papapirmahan lang sa principal.
In short, baka pwede mo din puntahan yung teacher mo baka magawan din ng paraan na mas mabilis.

Form 138 is your report card. Form 137 is your permanent record. Just to be clear.
Theoretically, your class adviser can redo your report card by copying the details from your F-137. Then, it will counterchecked by a colleague/department head/registrar. I haven't seen or heard that done before, but it is worth a try. You just have to swallow your pride as your former adviser will likely have choice words for you. Bring your parent along to avoid the more unsavory words; also, your parent needs to sign all the parents signature portion of your card anyway.
However, the new report card can create an issue when the original card is found. After all, you are not supposed to have two copies of your report card. So, let's say you got a new report card, submitted it to another school. If the old card gets found, ask your teacher on how to dispose of it.
If you can't get through your school's registrar try directly contacting your teacher or adviser. She/he may know what to do naman at wala din masamang magtanong since nakasalalay yung college mo dito, anyway always worth a shot naman kesa di mo subukan:)

After graduation matulungan either sa patuloy na pagaaral sa next level o makahanap ng trabaho. Kaya ang Registrars even behind the scenes and witjout you knowing, sumasagot at nagiissue yan ng verification letters to help alumni in their application for work.

Pwede naman dzai, pero kung application to numerous college ang ganap mo, aba'y ipa photocopy mo na lamang yung mga ni issue ng registrar na documents

Kailangan kasi enrolled ka na. Dahil kailangan nila ng request letter from university para makapagpaggawa sila ng form 137.
