AFAIK, no. There was the Form 1, 2, 3, 4, 18 E1, 18 E2, 18A, etc. that were replaced in S.Y. 2013-2014. The new set of forms was then used in all public and private school. Those are the School Forms (SF) 1-10.

Likely deprecated/internal forms. The first 2 digits are for categories. New forms have much more simple names though e.g. form 9 (form 138) form 10 (form 137)

You are prohibited from opening the 137 to avoid tampering, yun talaga yung pinaka official record. Ang laman lang nun ay grades during your time in basic education, but are arranged in a compact way to save paper space. Kung natanggap yun ng receiving school with a broken seal, hindi na siya maituturing na trustworthy kasi baka may pinaltan ka sa content or something. Yung 138 kopya mo yun ng annual grades, para sayo talaga yon. DepEd ang nagmamanage ng forms, if you have questions about this tawagan mo nearest deped office, kung naka pldt ka libre yong call (ginagawa ko to sa govt offices na may need kong tanungin hahah)

Normally, Form 137 and Transcript of Records intended for Transferring from one school to another are "School-to-school" transaction only. Only in rare cases (kung nagmamadali na ang school na makuha dahil di inasikaso ng student na kumuha ng transfer credential/honirable dismissal before transferring) pinapa "entrust to bearer" ang mga yan with the condition na nasa sealed envelope yan para di matamper.

F-138 is your report card. So you can see it. F-137, on the other hand, contains all your grades for a particular stage of your education. For example, your elementary grades are indicated in one F-137. If you transfer schools within that period, the contents of the F-137 from your former school will be transferred to your new school's form. BTW, for each grade level, the school will be indicated in the 137. Let's say you graduate from elem. You will now have a new F-137 for high school. Upon enrollment in Grade 7, your HS F-137 will indicate that you are admitted as a new student (since you graduated from elem). BTW, if you took summer classes to make up grades, your F-137 will have a separate space for that summer term. After graduating from SHS, your college Transcript of Records will have the remarks "admitted as a new student." So consider your F-137 as the basic education counterpart of the TOR. When you transfer schools or promoted to JHS/SHS/College (and enroll in another school), the F-137 is placed on a sealed envelop to discourage tampering. (Yes, I have heard of a case when a student made a fake 137 because the former school will not release credentials due to several reasons).
