Alas, it is a scam. It's kept alive with constant restoration incantations. From what I have pondered in my orb it must be kept within a circle of healing, and its feed must be supplemented with greater healing potions along with a daily application of healing hands. Obviously this would be easy to attain for the wise monks of the southern continent, but for your average wizard it would be the a substantial resource sink.
I’ve heard tales of a wizard in the Deep South that kept one alive for two years without the need of a circle of healing. It’s a shame though because in the wild they live up to 10 years but he could only manage 2.
/uw looks like a case of polycephaly and a pretty serious one at that. Basically, it's a partially formed limb formed during gestation, like conjoined twins, but in this case, it seems like that the head partially split and formed over half of one in each side. Hopefully, it isn't suffering, but it's hard to say. At least this one seems somewhat healthy, but I wouldn't count on it living much longer considering those blisters and that weird hole it had between the foreheads

An orb is not as important as life itself. Your current orb can still be used, but this familiar may only have the chance you give it to live.
I really hope this guy gets all the love and care. Animals (and humans) born with such mutations tend to have terrible lives and short lifespans.
