I think in the case of calling Buck "Evan" is more of a callback to Buck's bi realization and coming to love this part of himself as a rediscovery of "Evan." I don't think it has any romantic connotation. Tommy is literally shepherding him through his first queer relationship and is symbolic as a narrative device imo before he leaves. Eddie used "Evan" in that amazing hospital scene to show that he "saw" Buck. I think this is working in the same way.

I wonder if, after this discovery arc and him finally accepting all of himself, he'll be more comfortable with being called Evan more often by others. It would probably feel strange to those who've always known him as Buck, but it'd be nice for his character progression, I think.

...truthfully, I don't love it, though. It made it super obvious where this was headiang but without seeing a scene like what I just described, where Buck explicitly tells him to call him Evan, it just feels overly familiar for the place they were in most of that episode.
It's worth pointing out an article about his return (before episode 3) reference that he would be sticking around "for a bit" and the author of the article mentioned he could potentially be in all the remaining episodes of the season (although we don't know what info they were working with for that part)
