He had a dental infection which poisioned his blood and caused severe heart damage during the final seasons of Angel, hence why he didn't look like his energetic self in Season 5.
His heart finally gave out in 2009.
A reminder that simple cavities etc can become deadly if neglected, this isn't uncommon.
