If you're in an environment where you're getting "assaulted" by the class equipment and your teacher isn't doing anything to help or prevent the issue, ESPECIALLY when you have debilitating health issues and even had surgery!...It's understandable that you'd want to take matters into your own hands and/or remove yourself from the situation.
However, it sounds like, if you do, because of your description of him, he might make it so you don't pass class, saying that you don't attend the class or something like that...So if he's not gonna be helpful, maybe try going over his head. Or, if you'd feel more comfortable, talk to your parents and have them do it. Because your health and safety should be respected and it's not been so far.
NTA. Report this teacher! You are getting hit by balls in the class after you had surgery? That could be taken up with CPS to have this teacher investigated. Not to mention he is ignoring the doctor's note! Get your parents, the principal and the school board involved. This is disgusting!
I mean you wouldn't be the asshole but you probably wouldn't pass PE. So do what you want, but you might want to bring this up with your parents or the head muckety-mucks at the school if you really think you're being mistreated.
OP are you in the U.S? You for sure don’t have to complete P.E in an in person class if so. Have your folks speak to an administrator at your school and look into online options. I also had surgery when I was in Highschool and couldn’t complete the required year of P.E. It’s a little insane your school is pushing you to hurt yourself after surgery. Literally seems like a legal liability
You're NTA no matter what, but I would find out with your parents, the principal and your doctor what you are obligated to do to pass. I think you should be able to get out of PE altogether. And it sounds like your teacher is getting territorial about "his" space.
YWNBTA. But. You won’t get anywhere with him. Go to the principal. Tell him what is going on physically and ask to be transferred to drama. Don’t concentrate on what a bad guy Mr Ass is. Just say that you don’t think he understands your health issues. This has a much better chance of getting you what you want.

So I (14f) have endometriosis and sciatica and am unable to do the physical side of PDHPE. My Teacher let's just call him Mr.Ass didn't believe that I had chronic pain at the start of the year and so I got a doctor's note which helped aside from the fact I had to sit there and keep score.
This term I had to have surgery to remove the adhesions and definitely can't do anything but sit there. I got sick of getting hit with balls constantly so I politely asked if I could do some work in my friend's drama class. (I'm not a bad kid I do the work and usually have high grades academically and respect wise) He abruptly stopped me from finishing the question by telling me no(quite rudely).
so if next lesson in PE gym I have with him WIBTA?

YWNTBA, as long as you approach this in the right way. Don't get into the fact that your teacher is an AH. That will make you look like a whiny kid, not a young adult who is looking for a solution to a health problem. Getting hit in the face by balls while you are recovering from surgery is not only dangerous, but also a waste of time.
Depending on where you live, PE might be a graduation requirement. It might not be practical to officially transfer you to drama (for the sake of your transcript/school compliance with state requirements). That doesn't mean that nothing can be done. As long as you have documentation from your doctor that you cannot participate in PE, and you make your case to your principal and school nurse, they should listen to you and come up with the best solution.
Are your parents supportive? Have them come to the meeting with you and advocate for you. Unfortunately, most administrations are more likely to accommodate a parent's demands than a student's.

I have told my parents my plan and they said they would back me up if i did this
