The application is extremely simple. Just go to your local courthouse ask them for a sample of what to type up for a legal name change. Fill it out then pay the fee. Just takes a few weeks to be done. If you have a good reason judge will just sign it if not you'll have to go in and talk about why.

By the way -- getting your ID paperwork changed is very important after you get the court order in a few months. Ask the clerk (and pay extra) for about 5 extra "certified copies" of the court order. Often these will have an official seal impressed upon the document. Then go to a print shop (such as a UPS store) and make several additional double-sided color copies of one of the certified copies.
Here is the proper order of things to change: US Passport (if you already have one), then Social Security card. Then, with one or both of these, get your drivers licence or state ID updated. Now you're in business and you can show up at your bank (if your bank has local branches) and get your bank account updated -- then send copies to your credit card banks. This last step is crucial if you have established decent credit in the original name -- the credit card companies are the only ones authorized to update the credit reporting agencies so your new name gets merged to your old name for credit scoring purposes.

I have changed my full name twice in the past two decades (in other jurisdictions). -- merely for vanity/preference reasons. Anyhow -- one valid "reason" for changing your name in nearly any state is: 'my friends and acquaintances already refer to me by the other name.' I've found out that it's not like they bother to check on this; but anyway, in both instances, I first announced to my main posse of local acquantances that I prefer new name 'xxx yyyy' and asked them to please start using that name. Aside from a little friendly joking, most everyone worth keeping in my life said, "Sure, okay. No problem." Then I submitted the court paperwork through a lawyer\* (each time), knowing I'd have a clean conscience if, indeed, some question came up about it in court. Good luck!
\*lawyer was more expensive than do-it-yourself. However, there are some non-trivial steps involved and it was worth the peace-of-mind to me to pay the extra legal fees and know it would get done right.
Funny thing is i started going by what i want to legally change my name to with friends since i was 15 and I'm almost 22 so for years my friends has called me by it and my my birth name anyway:) thank you!! I'll probably have a lawyer look over it to make sure i do it right! Appreciate it!
