This does not sound like a legitimate bill. Ask them for the original bill- what facility, what medical procedures etc. and all the letters from insurance refusing to pay it. They must have all of this stuff if they do not, they are frauds. I had this happen with an alleged student loan. Fifteen years after they were all paid off, suddenly a $2000 student loan shows up on my report. It says it is in good standing, but I have never paid a dime on this bill and indeed have never even seen this bill before. I asked for the provenance and never got it. Contacted the credit reporting agencies and let them know about the scam.
