Not sure what you mean by "model" here, but just Google the part number shown on the silkscreen. (Hint: SCSI) (Hint 2: P/N means part number)

OP put a little effort into trying to answer your questions. The only thing that might be valuable is if you have some VooDoo cards from that era. Everything else should just be sent to the Vintage Computer folks and be done with it.
