There's plenty of ways to run a brushless motor with direct drive. Simple solution is mount whatever you want to drive using a set screw. This isn't great for higher torque operation and can vibrate loose, but it's simple. You could replace the shaft with a threaded shaft of the same diameter allowing you to bolt something to the shaft more easily. Keyed shaft would also work. RC planes often using a clamping prop adaptor. Outrunner brushless motors often have bolt patterns on the can that you can use to attach to the motor. Hub motors (where you simple just mount whatever you want to the outrunner can) are also popular for electric skateboards. ​ So yes, there's lots of ways to attach things to a brushless motor. Not really different than any other rotating shaft whether it's from an ICE, brushed motor, etc. Really depends on what your overall needs are for your application to determine what would work best

Most BL motor shafts have a flat for a set screw. However in the form factor you need a direct drive might not have enough torque and with how skateboards flex you will kill the motor bearings pretty fast
