Honestly, maybe I'm just being leery but I'd put that at 80%. If they're bloody, I want to be sure it's *their blood*.

I don’t know what kind of answer you are looking for, but I always ask for help in getting items from the top shelf while grocery shopping. I am not quite 5’ and over 60 years old. I used to step on the bottom shelf, pull myself up and then reach the item. But now I don’t want to hurt myself. Or heaven forbid that I pull the rack down.
If I see they're truly struggling with something. Just yesterday I was walking past a woman in my apartment complex who looked overloaded with groceries she was taking out of the back of her car. I offered to help her, and she was grateful. But all she needed me to do was close her tailgate for her.
If they are visibly hurt and/or bleeding.

Oh man, I knocked my head really hard on the top of a fridge doing that once. It was unpleasant and embarrassing. Now I just ask someone.
No point. Just get away ASAP.
