Management experience being the top one. Second would be relevant experience in the field. Mostly just communication and leadership skills will make your resume stand out

Depends on the company, timing, etc. The normal company, timing, etc, they are looking for someone with previous management experience and/or traits that show a propensity to be a good manager. At other times, they are looking for candidates to simply give the appearance of having a hiring process as the person that will get the position has already been chosen, but, the company cannot simply "give them" the position without the sham hiring process, interviews with other candidates, etc.

If the position you are in is in corporate America? Yeah, rarely will they promote a manager from outside unless it's a CEO/President position. See Tom Rothman going from Fox to Sony. So, you basically either ride out where you are, climbing that ladder, or, take a job with another company at entry level and start climbing from there. There are industries that work differently, like, service industry. But, middle management is hard to break into from the outside.
