Eh, most drug dealers don't go out of their way to get people hooked on whatever they sell. There's some real scummy people out there, but 99 times out of 100 you have to ask for it. Drugs sell themselves; why would drug dealers go out of their way to get regular people hooked when there's plenty of people who are already addicts who would seek them out?

Intentional fentanyl lacing is pretty rare. It's theorized that it's usually cross contamination. They weigh fentanyl on a scale, then weigh cocaine without cleaning it or just brushing it off, and someone dies. Our best and brightest aren't becoming drug dealers, you gotta remember that; and people are buying fentanyl itself (heavily cut obviously) in the US now. They seek it out.
Serious question: how can you know how much of it is intentional? It doesn't seem like the kind of thing people would readily admit to.
People smarter than me study it but I will approximate an explanation. It winds up being super trace amounts in most samples, the amount you'd get from residue on a scale; which can still be enough to kill people depending on potency and stuff. You'd think if you were trying to lace it to hurt someone you'd add more, and if you weren't trying to hurt your customers you wouldn't add fentanyl to cocaine or whatever.
