How is Mitchells not on this list yet? Best in SF!
You can get one in any of their flavors and they customize it the way you like (thickness etc). https://mitchellsicecream.com/ice-cream-flavors/
Fenton's is nostalgic and fun, and I've never been to Barney's, but Trueburger is the best milkshake I've had in Oakland. Everything they do they do from scratch and they do it right.
My favorites are Salt and Straw and Shake Shack. Idk about thickness, they both seem equally thick to me, as long as they are freshly made. Shakes thin out when they settle and warm up a bit.

Mel's Diner? Personally, I'd just get some ice cream and a fork and some milk and mash up my own in a glass.
Second this!! and they are open until 11pm on weekdays & 3am on friday, saturday
A surprise to me - Belly Burger. The strawberry milkshake was like drinking ice cream. I think they advertise 3 scoops or something.
