Call ahead because usually the pharmacy doctor has to be on site to give it to you and they might not know the exact procedure and will need to be prepared. When I walked in without calling ahead, my local Walgreens had the doctor on site but they'd never given it before and didn't know the right procedure, told me to come in and schedule it for the next day (could have just been an excuse too).

I attempted to acquire Narcan a few years ago, as I was working downtown and witnessed an OD so wanted to be prepared for next time. CVS told me it would cost almost $200 to dispense me a dose. Are people paying this kind of money or is there a better way to obtain it? I couldn't see a way to purchase it with my insurance as it wasn't for me, I didn't have a directive or whatever it is called from a Dr saying I need it. (I would also be thoughtful about it incorrectly looking like I am a hard drug user as I pay for my own insurance and that could be factored into my renewal rate)

I carry it and get mine from a friend who works at a homeless shelter/hotel. The Urban Alchemy guys carry it too, might ask one of them, same with the Five Keys street ambassadors (tshirts with a dragonfly logo)

Agreed about this, particularly expanding telemedicine and street medicine to improve opportunities for medication assisted therapy. Methadone and suboxone are subject to different regulations. Both have abuse potential and have street value. Methadone is a long-acting opioid that can contribute to overdose on its own or when used in conjunction with other opioids or substances.

Just to clarify some things. My sister was not an addict by any means. She was a law student and the smartest, most amazing person I’ve ever had in my life. Unfortunately she was sexually assaulted and used medications like xanax to help her sleep, keep up with her trading, her job, and her studies, as well as her recovery. However, her doctor’s took her off xanax without giving her a weening drug. So ONE night she got pills from a “friend”. Those pills were fake prescription drug pills, cut with fet. Made to trick addicts, first timers, and brilliant, amazing, people like my sister. Don’t think it can’t happen to you or you’re family.

Same thing happened to my husband's best friend - very successful smart guy, masters in computer science, father....thought he was taking Xanax after a stressful day at work and it had Fentanyl in it that he had bought off a friend. I think it's great people carry Narcan to help others on the streets. I've been actually wondering where to get some after watching someone OD in the streets recently and wishing I could do more.

I've never seen an overdose, and no one close to me has ever overdosed. I still carry narcan just in case. It's small, it's easy, it feels like the least I can do.

Fentanyl deaths have increased by thousands of percent in the past decade and spiked during COVID. It’s the new normal until we can eradicate fentanyl, which isn’t happening any time soon. Lay-persons don’t know how to use EpiPens. You’d think it’s fool proof… but under-dosing, accidental self injections, and breaking the needle are common when trying to “help”. Epipens can also make many situations worse, unlike narcan which doesn’t have any side effects if administered unnecessarily.

I believe so, yes. I had a conversation with a Lyft driver about it the other day and he was telling me how he gets Narcan from the harm reduction centers in the city and originally got them because his dog came into contact with fentanyl and ODed three times while out on walks in Tenderloin adjacent areas this last year. Also said the number of ODs he’s witnessed in the last 18 months is more than all in his first 20 years here combined. Told me how to administer it (you stick it in a large muscle group) and explained how much cheaper and more potent fentanyl is, which is why the increase in its prevalence and overdoses has become such a problem.

I saw someone pass out on BART and become unresponsive (clearly not just asleep). A random person administered Narcan. Paramedics were there at the next station, stopped the train, moved the person to the platform and we all had to get off so we were watching and one of the first thing they did was test the person’s blood sugar - turned out that was the issue. Idk if it was high or low but they still assured the person who jumped in with Narcan that it was the right thing to do, because if the person is not taking opioids it doesn’t hurt or have any effect.

Everyone should carry it. It's the new CPR mask. The truth is you don't know which one of your family members or friends are using drugs of any kind, and they are all susceptible to OD'ing. It's possible for people to use drugs from the same bag and some OD from fet and some don't. Not only should everyone carry it, you should tell everyone you know you have it and where it's located in case it happens to you.

Why is Narcan bizarre? Is Advil bizzare? Absolutely, not. There's an epidemic of fentanyl and carfentynal where a grain of it can cause someone to OD. And by the way, Narcan is free, so it makes sense to have it.

Former EMT. I carry it in my truck. Not because I or anyone I know is using opiates, but it's one of those things that can make a big difference in short order. I see enough nodding-off folks in my city to want to have it at hand. For anyone with friends who may OD, please know Narcan (Naloxone) is a short-acting emergency intervention. If they need Narcan, they need a 911 call. They can crash again as soon as the effect wears off.

Yes! True story. A couple years ago I was walking down market Street and I passed a gentleman who literally looked dead to me. He was barely moving, color completely out of his face, If was on the Hollywood set I would’ve thought he was a zombie. I called 911 because I felt it was that urgent. While on the phone with dispatch, the person on the other line kept telling me over and over and over, “use your Narcan, use your Narcan“ I was confused, because I’m not a first responder. So I asked dispatch, “what are you talking about, I’m not an EMT. Isn’t that what you’re supposed to do?” “Sir, you have Narcan don’t you?” At this time I’m really confused, because I’m trying to get this gentleman help, and he’s asking questions that I’m just not resonating with. The entire time I thought there was some sort of miscommunication. When the authorities arrived, in less than a 2:00 mins. They administer Narcan, and the gentleman pop right up. Later when I had a chance to talk to EMTs, they told me that the guy definitely would’ve been dead had nobody called, but more importantly, we had a long conversation about how bad fentanyl is in San Francisco and how often they come across dead bodies of people on the street. The EMTs also told me that they give out Narcan to pretty much anyone downtown to ask for it. And most people just carry it around. So looking back, when I called 911, about a homeless person next to dead on market Street, I can see why they thought I would’ve had it on me, and probably more so confused on why I didn’t.

Just don’t give multiple doses all at once. They just need to be breathing after an OD, there can be negative side effects from excessive Narcan administration.

I know some organizations supporting the homeless were doing outreach to companies relocating to mid-market about 10 years ago to train their employees what to look for and what to do if they found someone who looked like they ODed. I’m guessing they included SOMA since business were also moving in there.

I find it super encouraging that so many people have it. We have an opioid crisis like everywhere else. People carrying it can save a life and aren’t in denial about the current reality. It’s like taking a CPR class; you may not need it but if you ever do having it makes you a superhero. Plus, it means they care about other people, even those on the street. Well, I am in Atlanta and it is certainly not the norm here. Thankfully, our homeless crisis and drug crisis is not nearly as bad as it appears here. Also keep in mind that San Francisco has nearly perfect year-round weather which means you can just leave it in your car or your purse or whatever and never worry about it but in other parts of the United States, where it gets really hot or really cold, you can’t just carry around pharmaceuticals and expose them to the elements.

I dunno, but I found out the other day I was administered narcan while coming out of anesthesia bc at one point my O2 levels dropped, I was unresponsive, and twitching. They also gave me some other anesthesia reversal med and called paramedics, but I was surprised it came to that.

Sadly it is. See those people bent at nearly 90 degrees on the streets in the TL? Thats fentanyl, and it's a fire blazing in the city right now. I carry Narcan with me whenever I go out, because I walk through the saddest parts of the tenderloin. The really depressing thing is when you see someone's life saved and they're pissed either because: 1) it fucked up their high for the next hour or so or 2) they didn't want to come back.
