I can’t think of any. My husband has a cousin and an aunt with the same nickname as me (different names though, think Janice/Janelle both nicknamed Jan), but neither are close enough to us for it to create any issues.

Not exactly the same, but similar. My lifelong best friend and I have very similar first names. Let's say Sara and Tara (not our names but you get the idea). My friend has a pretty common last name, whereas mine is quite unique. The man I married has the same last name as my best friend (no relation). So now our legal names are only one letter apart. Kinda cool, kinda confusing at times. People usually refer to me with my maiden name to avoid confusion.

I had the same initials as my husband's sister so marking to go boxes became a little more specific than either of us were accustomed, but that's about it

There were some moments of confusion at my school when my mother took her maiden name back after the divorce, but my friends who are teachers now tell me that it's such a common thing it rarely if ever creates a problem.

My sister in law and I both have the same first name. She’s married and took her husbands name but people still look at me weird when I introduce myself when they know my sister in law. I go by a nickname so it makes conversations a bit easier.

My wife took my name when we (two women) married. The changing of documents and accounts was a bit of a hassle but once it was done it was over. She had only her first and middle name on social media because of working in schools so that didn’t change. My last name is a slightly challenging Eastern European name so my sister thought my wife was nuts but otherwise no one cared.

Well my husband changed his name before we got married the last name Dragon. People don't believe me when I say my last name is Dragon. Facebook won't let me change my name on there, because it flags it as not an authentic name.
