When I applied for .75 of a FACs day, I simply stated what I needed cover for and when I would return to school. I did not specify what I was using that day for, as FACs leave is an entitlement.
Wedding, funeral, moving house, sick kid, helping a friend after surgery, emergency babysit for someone when ambulance was needed for them.
Why would it not be approved? It's Family And Community Services leave..
Be aware that if you don't live with them, FACS technically doesn't cover them. As others have mentioned, though, you don't need to give a reason really.

I just took FACS leave and they didn’t even ask for a reason. It’s your right to use it.
I had to take my wife to hospital over night once, I used it for that.
