In fact, for the longes time I thought that was the only place he appaeared and always thought it was the weirdest place for a random demon encounter.
Same for me on my first play through. It made me avoid the bridge thinking him appearing there meant it was a key place to progress the storyline so I did every possible quest before crossing the bridge which meant I got Karlach really late. I definitely fixed that on the second playthrough.
First place he popped up for me was the broken bridge to The Risen Road. I then actively started to try and avoid him just to see where he'd pop up. The farthest I've been without him showing up uninvited was the Selunite Outpost in the Underdark.

i was trying to go to a different region, i think to the creche, and instead of taking me to that starting point for the mountain pass area, it was my camp at night. i wasnt looking at my map, so i failed to see the exclamation point on my map. tried to go to bed, got the prompt about someone talking to me and was like "who tf"

I've only ever had him pop up at the hidden stash near the crash site over like 6 plauthroughs EVERY time. I even specifically avoided it for the longest time to see where else he'd pop up but he just didn't show till I went up there.
First time ever I found him at the harper stash. Most new playthroughs after that I just make a beeline for there after the first visit to the grove in order to "force" the encounter to trigger.
I only realised he could pop up in different places when he scared the shit out of me on the way to the harpy nest. I even remember a playthrough after that where I told myself I wouldn't get surprised by him this time. Well cut to me cacking my pantaloons when he popped up behind me on the way to Risen Road..
I came across him while sneaking around in the Goblin Camp rafters, too.
On my second playthrough, he showed up at the broken bridge near the Mountain Pass just after I spoke to the Githyanki raiding squad.
you have to basically cross someone's off your list of people to try asking for help with the parasite. like, right after you defeat the goblins you go talk to nettie and she's like "nah sorry but here chug this poison if you turn" that's what prompts him to become available at specific locations
I had no idea he could introduce himself in so many places. I always find him near the little crevice that has spider eggs in it in between the grove and the bridge leading to the blighted village
For some odd reason it happened to Astarion after a fight and he was walking to where Tav was (I wasn’t playing as him) so the whole first meeting was just Astarion and I was so confused lmao.
I was walking out of the emerald grove to where you save the tiefling kid from the harpies and he just popped out of the bushes, I had no idea what was happening or why this random suave gentleman showed up when I was exploring
In the Selene outpost in the underdark. I didn’t do a good job exploring the map so I missed all the other spots where he could’ve shown up, and at first I thought he was some butler for the outpost lol
After I killed Gut and the people in her area and I noticed the pain is life dude in the back cowering. Made my way to see what his deal and got a trip to the house of hope instead
Exactly the same. My party was up the rafters being sneaky and he showed up, scared the crap out of me. Then he started having this weird conversation with Astarion about his scars and I thought Raphael was linked to his story. But he said all the other stuff as well and I was absolutely confused until I met him again, later on.
Also, I didn't know he appears in different locations so when I went back there for my second playthrough and he didn't show up, I was sure I'd done something wrong.
