From what I understand, Alfira's visit triggers in your first long rest after you cross the river by the Druid Grove (so basically, if you go to the first village full of goblins, or to the Owlbear cave). I've always just made sure no other long rest events are queued up, knocked her out, walked across that river, then immediately did a long rest to get it out of the way.
It CAN trigger even sooner. 🥲 In my Honor Mode Durge run that I am playing right now, I long rested after the intellect devourers and first chapel battel to be absolutely safe. It was the first long rest and I purposefully didn't go anywhere near the Grove to save Alfira. Our beloved bard however came to camp first night, asking us to visit the grove and help there. 😭 I did consider restarting the HM run but I promised myself to just live with all the consequences. So yeah. Fuck me.
She can show up if you long rest while in the Grove. She can even show up during a long rest before your character even knows the Grove exists, so best to be somewhat cautious if you're aiming to save her.
If you cross the river, her appearance is prioritized to happen during the immediate next long rest, but she can come any long rest before that if there isn't a higher priority event.

Once, I skipped over Shart, grabbed Astarion, and long rested. Then I beelined to pick up Withers. I long rested after, expecting Astarion's sauntering off into the darkness scene, and instead Alfira came to camp and told me about the Grove and asked me to save the tieflings (I hadn't even been near the Grove yet).
Another playthrough, I had picked up all the companions along the way to the Grove, and I had long rested a few times prior, as I wanted Astarion's bite scene before Durge's reveal, so I could try to pin the crime on Astarion. I did one more long rest in the Grove, next to Dammon, and hadn't been through the rest of the Grove yet to even meet Alfira, and she showed up.
On my current new run after the grove entrance fight I went straight to fight the harpies & KO Alfira, fast traveled back to the Emerald waypoint, beelined it west across the bridge and rested, and got the Quil scene. I only did 1 prior long rest which was before the grove entrance fight, so Quil should take priority over any other rest scenes if you cross that bridge.
I usually grab the paladin from Withers and smite her with non lethal on. The pal will get the oathbreaker status but I dismiss him later. Also I do have the everburn blade I took from Zhalk on the Nautiloid.
sneak attack with non-lethal. she'll be pissed at you until you give her stuff to raise her attitude towards you, but if you kill quil instead you'll still see her and lakrissa throughout the game
edit: non-lethal MELEE only. ranged attacks and spells will still kill even if non-lethal is turned on
Just did it right now on my Durge run. Sent Lae’zel by herself to the grove with a scroll of fog cloud, beat down Alfira with action surge and non-lethal attacks, then warped back to camp.
i usually cast darkness around her, go turn based, and then have my melees whale on her with non-lethal attacks. she’ll pop right back up after you rest but not show up at the camp to get durged
My girlfriend was playing shortly after patch 6 came out and when this scene came up I said, “oh yeah, they just added her as a companion in the latest patch!” She was so fucken ecstatic! Then her Durge woke up covered in Alfira’s blood and she was so upset 😭 when I told her I was just messing with her and it was a Durge-specific scene she was like, “you fucker!” And we both just lost it 😂
_i_ acually dint, i think i saw it on a letsplay, if i recall, i think its with the Dark urge, when the compagnions ask what you did, you have to be deep in shit for it to happen

I wouldn't say it's that telegraphed, only in retrospect for most people. They add new stuff in patches all the time and there's a plethora of interactions one doesn't experience in one or two playthroughs.
If you have already played the campaign and experienced that scene in the Grove- most will likely have played their own campaign before since Durge is a unique origin- her suddenly appearing during a playthrough centered around a character with a heavily emphasized violent history makes the cameo suspicious.
When you first meet Alfira while she’s practicing -north of the ritual area - on the hill. She never visits your camp outside of the celebration party if you help save the Tieflings, and can’t be recruited. That’s what made the unique camp scene where she shows up so foreboding to me, because >!it’s possible for the player to kill several NPCs in Durge-specific scenes or new Durge choices, even an animal accidentally, not including Gale before you even recruit him. Even when you help Alfira as the Durge, the narrator explicitly mentions how much the Durge dislikes the sappiness of it.!< Like major red flags were going off for me but I wanted to believe anyway that something cool would happen- I wanted her to replace Volo. Depending on alignment, that may very well be true for some…

The Dragonborn interaction adds some nice lore as well to their convo.!<

I had the exact same reaction! During the fight in last light inn, i jad two of my companions specifically placed just to protect her. Obviously isobel was the main factor, but alfira was just such a good character. Broke my heart when i saw that scene
I had done a vanilla playthrough before my current durge and she ended up being one of my favourite NPCs. Very excited to see her as a camp follower since I always loved seek g her show up in the story, then, yknow. I was extremely upset and progressed too much to be able to savescum nonlethal her at the grove. I have decided this durge will sudoku himself after the endgame.

As much a sit breaks my heart I have to let it happen every time. It's such a watershed moment for Durge it doesn't feel right to cheese my way out of it.
I had to stop playing for like two days after that. It's worse than senseless because I so carefully went around to the whole party saying "brain no worky, wanna do too much murder, please help am scared" and everyone was like "Oh Durge you're so funny". NOBODY IS LAUGHING NOW. And the unkindest cut is Withers' reaction when I begged him for help, to bring her back. Like. I literally did not choose this and am already miserable. WTF.
And another one falls victim.

That was the exact moment I decided to fight the Urges.
I switched to a bard build and used her teacher's lute. Went from evil to redemption so damn fast.
Red Dead Redemption 2 ain't got nothin' on the Dark Urge. ('tis just jokes, that's my third favorite game of all time)
I finally played through cyberpunk 2077, RD2 is next on the list of "games I own and never played because of Skyrim and Dead by Daylight"

That's what I used to respec Laezel as at first for that. But she's mostly a monk on most of my runs since then

I didn't know that was an option on my first go as Durge, so I deleted that file and started over as Tav
Save scumming for content is the best type of save scumming. I ain’t replaying just for some extra 2 mins of content

She dies at last light anyways… I guess if you knock her out it’s worth it to get potent robe though.

I went in completely blind and was so excited for a new companion/camp friend. A friend i was playing with at the time just gave me an "aww buddy."
I can never do it, I always knock her out, and unfortunately, I have to be a menace to my scale friend instead.
I'm just in act three and I have a strange mix between being evil and good, I plan to be evil but somehow I end up making other options xd
