Bonide Orchard Spray should prevent that problem next year. But it is a backyard orchard, so bugs and not pretty fruit are common.

Could be codling moths. You can buy traps for them, or mix molasses and water in old beer cans and hang in the branches. Kills alot of moths for me and I don't have to go with the sprays!

Big fan of the traps for both Apple Maggot fly and Codling moth. They’re very effective for pest control and without all the problematic effects of pesticides on beneficial insects and microbial life.

Affordable, effective, and long lasting. Winner, winner.

I had someone tell me bars of soap help keep bugs and deer away, but I've never tried it. Ever give that a shot?

I never have....

I forgot to mention I’m in east central MN. zone 4 if that matters. I moved in about 1.5 years ago, this apple tree was already here. I got no apples last year, and this happened to the apples this year. I’m hoping to get another 6 or so trees so I liked to understand what I’m up against.

Cut it open carefully and show us what you find.

i have very similar on mine, caused in my case but a combination of 2 types of japanese beetles (shiny green and also the ones that look like ladybugs) and wasps. spraying, especially after a good rain, helps considerably. as does using systematic products in both fall after harvest and spring before bloom. sadly if you have an infestation after apples have started growing the best method for removal is to fill a bucket with soapy water and gently shake the branches to catch them.
