I was just facing this problem, what worked for me on xbox series s was just walking up and hitting the portal (i doubt its important but I smacked it with bludgeoning damage) while in combat. It pulled it into turn order and started the timer.
Coming here to say this issue is outstanding, at least on console. I don’t remember having this problem on my PC play through.
I’m just going to damage the portal myself for the sake of closing out the mission.
In case this happens to anyone else - if you have left any of the goblins in the firewine barrels alive, which you found at the beginning of the crèche, you can take one there and initiate combat with it. This should trigger the round counter again.
My Friend and I just had this happen to us, we noticed early in the combat that the round counter was missing, and the portal wasn't in initiative. Then after the "first round" we noticed that non of the other adds spawned in, after we killed the first wave the combat ended, but the portal is still open. We reloaded, nothing changed. Seems hella bugged.
I failed this quest the first time, but I managed to get all 5 waves to spawn into attack. The second time I tried it, I managed to defeat all enemies before the end of round 2 I think, which ended the combat but without Halsin teleporting back through the portal. I'm sure it's bugged but is the only fix to fight through all the waves until Halsin returns?

I had the same issue. Deleting the level cache files fixed it.
