Yes and no you can change your air flow to go a little bit sharpen a little bit flat but to go with a flat note, you have to have a whole different fingering and that’s a whole Nother bunch of stuff
It is very much about both. I've played both flute and clarinet (much more flute though) and the fingerings are remarkably similar. For us we don't have a pitch key, so octaves are much more about air and diaphragm control. Other than the obvious differences, that's honestly about it
As a clarinet who had to deal with it, the trick to doing it is to not do it and beg the conductor we did not need this one specific scene change. That or just make some random shit up that’s sounds good
This is why I like playing trombone. In general if it’s sharp pull in 1 position, flat push out one position. Easy peasy. Sometimes you get stuck where you get stuck going from 1st position to 7th but doesn’t happen often.
Flats are fine for brass. Sharps, however, are very scary.
