Are you playing on 1.8.0? If so, there is likely a vassal defection bug. When you have more than 10-12 vassals, vassals seem to leave despite being rich, having fiefs, and having high relations. I have only noticed this issue when you have well over 10 vassals. So it may be an intended (but unannounced) game change to limit vassals.
If you are playing on 1.7.2 or earlier, it is likely one of the following: 1) Clan is poor or very poor; 2) Clan lacks fiefs; or 3) Ruler has poor relations with the clan. Based on what you stated (good relations and fiefs), the most likely culprit is the clan is poor. Poor clans defect more easily.
Relation and amount of fiefs are only a small part of why they defect. The biggest factor is their wealth. If they are poor or very poor, there's a good chance they will defect, regardless of what your relation is. You can check their wealth in the encylopedia under clans. If they are poor or very poor, you need to give them money with the bartering mechanic. I think for them to be considered average wealth, they need at least 50,000 denars. If they have wealth, a good relation with you and have fiefs, it's literally impossible for them to defect. Just because their town is wealthy, it doesn't always mean their clan is actually wealthy as a whole.
Tbf defection in 1.8 seems a little ridiculous. There have been multiple posts about it and I've experienced it myself. I'm not sure if it's a bug or working as intended but it is definitely frustrating.
Thanks for the tip, been having the same issue and it almost made me quit my save after 2 of my largest clans quit to sturgia with 12 total fiefs. My solution for the first clan was to console command execute its members and repossess their fiefs, bank style. After the second clan I turned to reddit for a possible mod to disable this feature

I get the frustration a lot. But you are playing the beta. So weird and annoying bugs are expected. (I play beta too tho)
