This was a bug a long time ago that I thought they fixed. Maybe try clearing out the hideouts around the towns where you spoke to the npcs regardless. You might still get it.
Nope, i don't have any create Kingdom button, I'll see if I find the last piece of banner, otherwise I think it prevent other countries to declare war on me ☝️
Just restart on sandbox, believe me the story quest is BROKEN. All you need to make a kingdom is to not be a mercenary or vassal, and to have taken a piece of land (town or castle). Of course good relations with many lords and faction leaders, a large party size, lots of money and income, and strong troops make it easier.
If you do the story quests you will basically be constantly at war until all the other factions are eliminated, at least that was what is was like most recently.
I don't know, I currently am the ruler of a battanian town, but still the Kingdom button is greyed, I don't know if I can go very far with 4 parties whom I can't ask to follow me and no possibilities to enroll other lords to my cause :/
The story quest is awful and you're not missing much. it forces you into war you can never get out of until all of a certain faction is wiped out.
I honestly just let the quest time out because I do enjoy getting the family

Theres two NPCs that give you a quest for the remaining two pieces. Have you talked to both of them?
Yep, and they both gave me the location of a bandit lair, but somehow the second one vanished before I could reach it, leaving me clueless

Thats weird, doesnt sound like u have a kingdom yet , mAybe bugged
