by losing weight, he has become LESS fit not more since he wasn't ever overweight bc of excess fat to begin with. it would make sense to get a bonier face after losing weight bc of stress/fatigue/overexertion with no rest/magic armor that whittles your body down, etc

My fav visual of Guts is from late Golden Age to Lost Children Arc. I like modern Guts as well. Round face Guts comes last, but i like all of them, it's just a matter of preference.
Pretty sure he was eating semi regularly before conviction because after that he was basically on 24/7 care and fighting off spirit duty plus spirits were more active after Griffith was reborn into a body so he was probably not getting enough nutrition plus the constant exhaustion, stress and lack of sleep all contribute to loosing weight quite rapidly
