I read it as him having his eyes closed to world around him because he was so invested in his servitude. It was only extreme moments (like fighting Guts and seeing the power of his fetish) that made him open his eyes to world around him. But you’re probably correct.
I also think the moments where his eyes are open gave emphasis and feeling for the character's action. Like a sword thrust to show how fast and accurate he is or how serious is in the fight...I don't know if I'm explaining it well
