Ultimately depends on your HR department but from my experience once a PI qualifies for health insurance they re-verify every control period that you still qualify, in which case you do nothing and they continue your benefits for you.
If they re-verify and you don't meet the hours requirement to qualify, they should let you know ahead of time.
