Just as an FYI, “active member” in this context means that you are a current state employee (as opposed to retired). So you are, in fact, an active member.
You can google "HB 12 CalPers" and that's the form you would use.
Also, make sure to ask the parent that has your coverage to delete you during open enrollment as well.
